AI Incident Response Lead
Also called AI Incident Manager · AI Incident Investigator · AI Safety Incident Responder
An AI Incident Response Lead coordinates the response when a deployed AI system fails or causes harm. They contain the damage, find the root cause and make sure the same failure does not return.
What they do
They own an incident from first report to closure. That means triaging alerts and user complaints, deciding severity, and pulling in engineering, legal, policy and support. They direct containment, such as rolling back a model, tightening filters or switching a feature off. They run evaluation tooling and stress tests to reproduce the failure and narrow down the cause. They keep a clear incident record, brief leadership in plain language, and write the corrective plan. Between incidents they run drills, tune escalation paths and review near misses.
What a strong hire delivers
In the first months, a strong hire gives the company a working incident process where there was only habit. Severity levels are agreed. Owners are named. The first real incident is contained faster than it would have been before. Root causes are written up honestly, and corrective actions are tracked until they are done. Leadership gets a clear view of risk and recurring weak spots.
How you can tell they are good
Ask them to walk through a real failure they handled, including what they got wrong. Strong people separate containment from root cause, and say what they did while the facts were still unclear. They can explain uncertainty without hiding behind it. Ask for an incident record they wrote, with details removed. Be wary of anyone who talks only about process, or only about models.
Why it is hard to fill
The title barely exists yet. The work usually sits inside reliability, trust and safety, risk or ML engineering roles, so job titles tell you little. Few people have run a genuine AI incident, and fewer have done it across engineering, legal and leadership at once. Candidates tend to be strong on one side, either incident command or model behaviour, and rarely both. Demand is high and the pool is small.
Where to find them
Look at site reliability and incident management leads who have moved close to ML systems. Also look at safety engineers, applied ML engineers who handled production failures, security incident responders, and risk or assurance specialists. Frontier labs, regulators and insurers are the main employers today. Some of the best people are in evaluation or red teaming teams, doing this work without the title.
Indicative pay
Not enough published evidence yet to give a fair range for this role, which is common for work this new. We benchmark it for you as part of a search.
Interview questions for an AI Incident Response Lead
- Tell me about an AI system failure you helped contain. What was your role, and what did you do first?
A strong answer names a specific incident and their own actions. It shows quick triage, clear ownership and early containment before the cause was known. Worry if they only describe the team, or claim no mistakes.
- How do you decide the severity of an incident when you do not yet know how many users were affected?
Look for a sensible approach under uncertainty: worst plausible case, fast sampling of evidence, and clear rules for escalating. They should revise severity as facts arrive. Worry about waiting for certainty or defaulting to alarm.

Job description you can post
We are hiring an AI Incident Response Lead to take charge when our deployed AI systems fail or cause harm. You will coordinate the response, contain the damage and make sure the lessons lead to real change. You will work across engineering, policy, legal and leadership.
What you will do
- Lead response to AI incidents from first report to closure
- Assess severity and direct containment actions
- Work with engineers to reproduce failures and find root causes
- Maintain clear incident records and communicate with leadership
- Write and track corrective plans until they are complete
- Run drills and review near misses to improve our process
- Help shape how we monitor and escalate AI risks
What you will bring
- Demonstrated experience leading response to production incidents
- Working knowledge of how ML systems fail and how to evaluate them
- Ability to coordinate engineering, legal and policy teams under pressure
- Clear writing, with examples of incident records or post-incident reviews
- Sound judgement when evidence is incomplete
- Experience with incident systems and evaluation or stress-testing tooling
